Bioidentical vs Synthetic HRT: What's the Real Difference?
Bioidentical vs synthetic hormone replacement therapy — molecular differences, safety data, and why the best hormone doctors choose bioidentical estradiol and progesterone.
"Bioidentical" means a hormone has the exact same molecular structure as the one your ovaries used to make. "Synthetic" means it does not. That difference matters more than the marketing suggests.
The molecules
- Bioidentical estradiol — identical to ovarian estrogen. Delivered via patch, gel, cream, pellet or injection.
- Conjugated equine estrogens (Premarin) — extracted from pregnant horse urine; contains estrogen molecules not native to humans.
- Bioidentical progesterone — identical to ovarian progesterone. Calming, sleep-promoting.
- Synthetic progestins (medroxyprogesterone, MPA) — not the same molecule; carry different risk profiles.
Why the difference matters
The 2002 Women's Health Initiative scare was driven largely by Prempro — Premarin plus the synthetic progestin MPA. Subsequent data on transdermal bioidentical estradiol with oral micronized progesterone shows a markedly different risk profile, with no increased breast cancer signal in most studies and lower clotting risk than oral estrogens.
What the best hormone doctors prescribe
Transdermal bioidentical estradiol + oral micronized progesterone is the default modern regimen, often paired with low-dose testosterone for women and vaginal estrogen for genitourinary symptoms.
Compounded vs FDA-approved bioidenticals
Both can be bioidentical. FDA-approved versions (Estrace, Climara, Prometrium) are bioidentical and rigorously dose-standardized. Compounded options shine when a patient needs a dose, ratio or delivery method that isn't manufactured commercially.
