Is BHRT Safe? What the Latest Research Actually Says

Is bioidentical hormone replacement therapy (BHRT) safe? A clear, current look at the research on breast cancer, heart disease, blood clots and dementia risk.

The short answer: for the vast majority of healthy women starting hormone therapy within 10 years of menopause and under age 60, modern bioidentical HRT is safe — and often protective. Here's what the current evidence actually shows.

Breast cancer

Estradiol alone (in women without a uterus) does not increase breast cancer risk; the WHI estrogen-only arm actually showed lower risk. The risk signal in the combined arm came from the synthetic progestin MPA, not from estrogen. Oral micronized progesterone — the bioidentical option — has not shown the same signal.

Heart disease

Starting hormone therapy in the "window of opportunity" (within 10 years of menopause) is associated with lower cardiovascular disease and all-cause mortality. Starting more than 10 years out, the picture is more mixed.

Blood clots and stroke

Transdermal estradiol (patch, gel, cream) bypasses the liver and has not shown the clot risk associated with oral estrogens. This is why the best hormone doctors default to transdermal delivery.

Brain and dementia

Estrogen receptors are densely concentrated in the brain. Observational and emerging trial data suggest hormone therapy started in the menopausal transition may protect against cognitive decline and Alzheimer's, especially in women with the APOE4 genotype.

Bone

HRT is the most effective intervention we have for preventing post-menopausal osteoporosis.

Who should be more cautious

Active or recent breast cancer, active clotting disorders, untreated severe hypertension, and active liver disease are reasons to individualize carefully. A skilled hormone doctor will work through these with you rather than refusing reflexively.
